Organisations are increasingly working together to integrate services, care and support for the citizens and patients they serve, across Cities, Towns and Boroughs (Places in Place Partnerships) and whole regions for example South Yorkshire (Integrated Care System).
Those who deliver direct Health and Care are also working together in groups for example Hospitals (Provider Collaboratives).
The purpose of this Functions Decision Map is to give a high-level summary of how South Yorkshire Integrated Care Board works and is working with other Organisations, Committees, Partnerships and Groups to deliver the four key purposes of an ICS.
