Welcome to the September edition of the NHS South Yorkshire Primary Care Bulletin.
We’re really pleased to have joined up with the NHSE team to get out to the four PCNs taking part in the national PCN test programme. The level of enthusiasm and willingness to develop innovative models of care for local communities was really encouraging and we’re looking forward to joining the 22 successful test sites from across the country at the launch event next week.
It’s becoming increasingly clear that the programme is about co-production, with flexibility for the PCNs to test what they believe is best for their patients. A wide range of interventions will be tested across the 22 sites resulting in opportunity for shared learning, the development of a range of metrics to measure impact and the creation of a framework that describes the key components of modern general practice.
Importantly as an ICB we are committed to ensuring that the benefits and learning from our local test sites is shared across the whole of SY so that all PCNs benefit.

Invite – South Yorkshire Primary Care Race Equality Network – 26 Sept
NHS South Yorkshire ICB’s Race Equality Network for Primary Care staff is open to all staff working across Primary Care including General Practice, Optometry, Pharmacy and Dentistry, and welcomes attendance from allies wanting to improve their understanding of the issues and challenges.
The Network is even more pertinent given recent events with the racism and discrimination that surfaced through the civil unrest in early August. We hope this meeting will provide insights and resources to help people to safely challenge discrimination and abuse, promoting wellbeing and safety.

Sheffield Teaching Hospitals new Electronic Patient Record system
Please be aware that patients will not be able to book, change or cancel an appointment at Sheffield Teaching Hospitals on e-Referrals Service between 10-14 October.
As you many have heard, Sheffield Teaching Hospitals (STH) will be launching a new acute Electronic Patient Record system called the Connect EPR on Monday 14 October 2024, replacing Lorenzo and some of their other acute systems. The process of switching off some current systems and transferring patient data across to the Connect EPR will begin on Thursday 10 October. During this ‘cutover’ period, they will be updating their e-Referrals Service.
If patients are referred during this time, it won’t be possible for patients to book, change or cancel their outpatient appointment.
GPs and Consultants will still be able to triage referrals as normal and make necessary changes. However, if patients log in to book their appointment during this period, no available slots will be visible. Patients will need to select the option ‘Defer to Provider’, and our admin team will then book the appointment for them and send them a letter to confirm this.
The e-Referral Service will be back online from Monday 14 October. STH will then re-publish the appointment slots which will show online just as they did before.
Please note that this is the only action you and your patients will need to take and there will be no changes to current processes for booking appointments moving forward.
Introducing the Pumping Marvellous Heart Failure Awareness Campaign and the Heart Failure Academy
The Yorkshire and Humber ICBs are launching two key initiatives to improve heart failure awareness and education across our region.
Pumping Marvellous Heart Failure Awareness Campaign:
• September: Social media campaign focusing on heart failure signs and symptoms, using targeted algorithms to reach specific demographics within the community
• October: Distribution of BEAT HF posters to all GP surgeries across Yorkshire and Humber, and credit card-sized symptom checkers to be given to heart failure patients. The NICE-endorsed BEAT acronym simplifies symptom recognition, and the posters should be displayed where they will be visible to patients.
Heart Failure Academy Launch (October/November):
This educational website and e-learning platform aims to enhance heart failure knowledge and confidence among healthcare professionals. Features include:
Accessible Education

Interactive curriculum including modules on diagnosis, treatment, management of HF.

Content suitable for all colleagues with an interest in HF.
Expert Knowledge
-
Developed by specialists both locally and nationally.

Incorporates existing best practice and the latest innovations in HF.
Collaborative Learning

Flexible delivery, including webinars, workshops, podcasts, videos, case studies, and face to face delivery.

Peer learning through an online forum.

Three healthcare teams from across South Yorkshire have been announced as representing the region at the prestigious NHS Parliamentary Awards next month.
This year, ten award categories cover exceptional care and health innovation across healthcare services.

PRIMARY CARE SUPPORT
DAYLIGHT – a digital app to help patients deal with anxiety and worry
Just a reminder that the Daylight App for Generalised Anxiety Disorder went live earlier this month.
Daylight is a free, evidence based digital app treatment for anxiety and worry. It has been developed by Big Health who are the makers of “Sleepio” an app to treat insomnia. Daylight has been rolled out and used to benefit patients in Scotland for some time and is now being provided to South Yorkshire.
Daylight delivers CBT, the NICE 1st line recommended treatment for anxiety, via a smartphone app that patients (18+) can access instantly, in their own time and at no cost.
Patients do not need to be referred or prescribed the app, they simply need to visit www.trydaylight.com/nhs to get started. For healthcare professionals, it provides them with another non-drug option when considering whether to start patients on medications.

Primary Care Children and Young People’s (CYP) Eating Disorders (ED) Decision Tree
Please see here the Primary Care Children and Young People’s (CYP) Eating Disorders (ED) Decision Tree. The CYP ED Decision Tree tool has been developed by North East and Yorkshire Regional Mental Health Team, in collaboration with their ED and Primary Care Clinical Leads, and the North East and North Cumbria Provider Collaborative.
The tool explores what steps to take in primary care when a young person presents with restricted oral intake and provides signposting information. The tool includes a blank page at the end for recording local service details (text can be added to PDF documents using a PDF editor). The up-to-date version will be saved on the FutureNHS Platform here. There are also a range of training recordings available including disordered eating, early engagement and intervention, getting it right first time, and more.

Free accredited training in health inequalities and patient experience
The 123 Approach is a free online training course helping to equip colleagues with the knowledge and skills needed to help tackle health inequalities and improve patient experience across the cancer pathway.
Developed by the Cheshire and Merseyside Cancer Alliance and Cancer Academy, the training aims to empower you to make an impact within your own role, offering continued support beyond the course.
Designed in collaboration with people who have lived experience of cancer and NHS colleagues working across the cancer pathway, the course is fully funded, CPD accredited, and aligned to the ACCEND framework. It should only take between 60 – 90 minutes to complete.
