NHS South Yorkshire has published an independent investigation report, on 5 October (today), into the care and into the death of Yusuf and the care and treatment he received.

Niche Health and Social Care Consulting were commissioned to undertake an independent investigation into Yusuf’s whole care pathway from his first contact with health services on 15 November 2022, to establish the facts on which clinical decisions were based and determine whether there are any lessons to be learned from the case in relation to the provision of and access to emergency and inpatient paediatric care.

Dr David Crichton, Chief Medical Officer at NHS South Yorkshire said: “The report responds to concerns raised in the treatment and care of Yusuf, a young boy who tragically lost his life after a short illness, our thoughts and condolences are with his family.

“We would like to thank the family and members of staff who inputted into the report. We acknowledge the findings from the independent investigators, who looked at the care and treatment across all health services involved and fully accept the recommendations in their report. While the report concludes that they would not have changed the course of events for Yusuf, it is important that our health services focus on actions to further improve patient care.

“Significant steps, have been, and will continue to be taken to learn from this case.”

The report has now been published on the NHS South Yorkshire website. Organisations involved in the report will also publish the report on their websites.
