Welcome to the October edition of the NHS South Yorkshire Primary Care Bulletin.       

As the winter vaccination programme kicks off we wanted to say thank you to all practices and pharmacies who are helping to deliver this and ensure those who are most at risk of getting seriously ill are protected as we head into winter. In this bulletin we have shared some easy read information for people with a learning disability or severe mental illness which you can find in an article below. 

We also wanted to draw your attention to a programme of work of the South Yorkshire and Bassetlaw Cancer Alliance. They have recently announced that their lung screening programme has successfully found an incredible 511 cases of cancer since it was launched as a pilot in Doncaster in 2021. The programme has since expanded across Bassetlaw, Rotherham, and Barnsley – and now those most at risk of lung cancer will be offered a free NHS Targeted Lung Health Check as the programme launches in Sheffield. Lung Health Check Programme finds 500 cancers and Sheffield launch is underway 

lung screening.pngA lifesaving screening programme started as a pilot in Doncaster in 2021, and has since expanded across Bassetlaw, Rotherham, and Barnsley – and now those most at risk of lung cancer will be offered a free NHS Targeted Lung Health Check as the programme launches in Sheffield. 

An incredible 511 cases of cancer have been found to date, with 402 being lung cancer. What’s more impressive, is that 75% were caught at an early stage and 74% of these patients were suitable for curative treatment.

New funding boost for green social prescribing in South Yorkshire 

people-2567915_1280.jpgNHS South Yorkshire has received further national funding to support green social prescribing and improve the mental health and wellbeing of people living across South Yorkshire.

Cancer Innovation Grants programme for 2024/45 now live! 

people-2557396_1280.jpgThe South Yorkshire and Bassetlaw (SYB) Cancer Alliance, in collaboration with Health Innovation Yorkshire & Humber, have now launched their Cancer Innovation Grants programme for 2024-25. 

Individual grants of up to £30,000 are on offer to successful applicants within the region’s health and care services, including primary care, secondary care, and Voluntary, Community and Social Enterprises (VCSEs). Collaboration across organisations and sectors (including industry) is encouraged.

Help Us Support Patients with Early-Onset Type 2 Diabetes in South Yorkshire

SY Diabetes Website_Artboard 2.png We have launched a new campaign, Your Diabetes, Your Journey, aimed at supporting individuals under 40 diagnosed with Early-Onset Type 2 diabetes.

 Research has shown that younger patients are more likely to experience denial or delay in addressing their diabetes, which can lead to rapid escalation of health complications. We kindly ask for your support in raising awareness among your patients to help prevent these risks.

Sheffield specific service - Changes to referral process for the Ambulatory Wound Care & Lymphoedema Service

nhs-doctor-using-a-computer-540x334-e.jpgWe are pleased to introduce a new joint referral form following the amalgamation of the Ambulatory Wound Care (AWC) and Lymphoedema services. This will streamline the referral process for all referrers and will enhance the communication, improving patient care by ensuring the patients are seen by the right nurse at the right time leading to a more efficient service.  

Key Changes 

  • The new joint email address sth.awc-lymphoedema@nhs.net will replace the previous separate email addresses. 

  • The new form includes the referral criteria for both services and provides clear guidance to ensure referrers are aware of what information is required. 

  • The new form consolidates the referral process for both leg ulcers and Lymphoedema patients. 

  • The form is now available in a printable format and on SystmOne under AWC folder in Communications and Letters- "Referral Forms" > subcategory "AWC & Lymphoedema.”

Please find the new referral form attached to this email. From 1st October 2024, please use the attached referral form. Detailed instructions for completing the form are included in the attachment. Hepatitis C & Primary Care

Doctor-8881542_1280.jpgAre you a GP in South Yorkshire? Are you passionate about helping to reduce health inequalities? If so, are you interested in becoming a Hepatitis C Champion and playing a part in the national Elimination Programme? 

The Hepatitis C Operational Delivery Network (ODN) for South Yorkshire, Bassetlaw and North Derbyshire, based at the Royal Hallamshire Hospital in Sheffield is looking to recruit 2 to 3 Hepatitis C GP Champions to work in the Sheffield, Doncaster, Barnsley and Rotherham areas to support local efforts in South Yorkshire to achieve elimination. This is a flexible, paid role consisting of between 2-4 sessions per month or up to 2 days a month equating to 4 GP sessions (approx. £380 per session).

All we are asking is that you be a point of contact for us to send information and updates to your practice or PCN, which you could then hopefully share, and that you might find opportunities to raise awareness amongst your colleagues about improving the care of young adults 18-39yrs with T2Diabetes.  

We will share a range of resources with you and add you to our South Yorkshire Diabetes Newsletter. We can also provide you with data about your own practice or PCN in relation to the number of younger T2D patients, and the completion of their Key Care Processes, which can be shared with your practice managers, partners, and clinical staff.

Community Pharmacy Advanced Services Workshop  

Community Pharmacy South Yorkshire, in collaboration with South Yorkshire ICB and the South Yorkshire Community Pharmacy Forum hosted a workshop on Sunday 22 September aimed at supporting Community Pharmacies to elevate their impact and maximise their delivery of advanced services.  

The workshop included sessions on the Pharmacy First, Hypertension Case Finding and Contraception services, as well as building effective relationships between Community Pharmacies and GP Practices.  

The workshop was attended by Community Pharmacy colleagues from across South Yorkshire, with feedback that it had been both helpful and informative. Future workshops are currently being planned for 2025, with details to follow.

South Yorkshire Stroke Conference

Non-medical prescribing (NMP) / Independent prescribing (IP) support and development

South Yorkshire Primary Care Workforce and Training Hub run regular lunchtime clinical updates to support your prescribing practice. They are open for all NMP / IP (including those who are not yet prescribers)  who work in primary and community care. Each topic is tailored to clinical work and delivered by local experts. They are a great way of maintaining your prescribing CPD that is specific to your day-to-day work.

DigiBete App & Self-Management Platform  

Free clinically approved support and resources whenever patients need it.  

DigiBete is a video platform and app and is a one-stop shop for young people's diabetes management. Founded by families living with diabetes, it is a social enterprise funded by NHS England with all content clinically approved by the Diabetes Team at Leeds Children's Hospital.  

We now have a South Yorkshire Clinic code, allowing South Yorkshire patients free access to the App, which will include a range of films, supports, resources, communications, appointments, notes, goals food diary and local information.  

Your Clinic Code is:  DVIKG (all letters)

Cambridge Diabetes Education Programme, FREE for staff across South Yorkshire  

CDEP is an online training platform which all levels of staff can access, both registered and unregistered.  

CDEP is designed to: 

  • boost people's knowledge, confidence, and guideline familiarity. 

  • be done in 'bite-sized' chunks fitting into your busy life. 

  • accessed from a computer, mobile phone, or tablet. 

Each topic takes between 15 minutes to 2 hours to complete depending on the topic.
